Referrals can be made to the COC by state agencies, private providers, or other individuals in the community.

For youth with Medicaid, please contact the Phoenix referral system at 1(888) 549-0820 and request the COC as your provider.

For all other non-Medicaid referrals, please contact our office directly. Click here to find the appropriate office contact based on the county the youth resides.

After the initial screening and assessment, if the COC is not able to assist a family, the family and the COC, together, will discuss options for alternative services.
