Intensive Care Coordination
COC provides intensive care coordination (ICC) to children and families. ICC is an effective process of planning that builds on the collective action of a team and helps the family reach their goals for the future. This team includes youth, families, natural supports, and professional service providers.
Through monthly team meetings, COC will develop a community-based plan of care that includes strategies to ensure youth and their families can experience success in their communities, homes, and schools. If therapeutic admission is necessary, we strive for solid discharge planning that is short term and includes a return to the home and community.
COC augments existing resources to ensure:
- Youth have access to a full array of services that allow them to be served in the least restrictive, most appropriate setting
- Families have the necessary supports that allow them to experience continued success long after wraparound care coordination has ended
