Referrals can be made to COC by state agencies, private providers, or other individuals in the community. 

Parents/Legal Guardians

• If you wish to apply for services for your child, please fill out the application [PDF].  COC will review the application and conduct an initial screening and assessment. If a child is determined to be ineligible, COC will work with the family to explore options for alternative services.

Referrals for Youth with Medicaid

• Visit the Phoenix Provider Portal or contact the Phoenix referral system at 1-888-549-0820 and request COC as your provider.

Referrals for Non-Medicaid Youth

• Contact one of our offices directly based on the county in which the youth resides.
